Background
Phase 3, open-label, randomized (2:1) GIMEMA ALL2820 trial. N=236 adults (median age 55 y, range 24–82) with newly diagnosed Ph-positive ALL. Tests chemo-free ponatinib (3rd-gen BCR::ABL1 TKI) + blinatumomab (CD19 BiTE) vs imatinib + chemotherapy frontline.
Results
Interventions and follow up: Arm A: ponatinib + blinatumomab (n=158)
Arm B: imatinib + chemotherapy (n=78); crossover to Arm A allowed for persistent/reappearing MRD
Primary endpoint: EFS
mFollow up: 23.7 mo
Results: 24-mo EFS: 86% vs 67% (P<.001)
MRD-negative survival: 68% vs 42%
CHR end of induction: 94% vs 79%
Molecular response after blinatumomab consolidation: 71% vs 49%
Molecular response after crossover: 64% (20/31)
OS: 90% vs 85%
Cumulative incidence of relapse: 5% vs 13%
HSCT in CHR1: 19% in Arm A

Conclusions
Frontline chemotherapy-free ponatinib + blinatumomab was superior to imatinib + chemotherapy for EFS, with deeper molecular responses and few patients needing transplant in first remission.
Key Limitations
Open-label design; median follow-up ~2 y, OS difference not yet separated (90% vs 85%); crossover allowed in control arm may dilute OS; role of HSCT and optimal duration of therapy remain undefined; long-term ponatinib toxicity not captured yet.
Clinical Context
Builds on single-arm GIMEMA/MD Anderson ponatinib-blinatumomab data and the GIMEMA ALL2820 chemo-free concept. Provides randomized evidence supporting a chemotherapy-free frontline approach in Ph+ ALL; guideline positioning not yet established at time of drafting.
