Background
Phase II, open-label, single-arm study. 60 patients with R/R marginal zone lymphoma (MZL; nodal, extranodal, and splenic subtypes) after ≥1 prior anti-CD20–containing therapy. Ibrutinib is a BTK inhibitor with demonstrated activity in B-cell malignancies. PCYC-1121 established ibrutinib as the first BTK inhibitor approved for MZL.
Interventions and follow up
Regimen: Ibrutinib 560 mg PO once daily continuously in R/R MZL after ≥1 prior anti-CD20 therapy

ORR (primary): 58% (95% CI 44–71%)
CR rate: 3%
Median DoR: 27.6 months
Median PFS: 15.7 months

Adverse events
Overall: Grade ≥3 AEs 47%; treatment discontinuation due to AEs 17%
Cardiovascular: Atrial fibrillation any grade 12%; hypertension 8%
Bleeding/infection: Major hemorrhage 12%; grade ≥3 infections 15%
Conclusions
Ibrutinib achieved a 58% ORR with durable responses (median DoR 27.6 months) in R/R MZL, establishing BTK inhibition as an effective treatment strategy for this histology and supporting FDA approval.
Key Limitations
Single-arm phase II with modest sample size (n=60) and no comparator, limiting relative efficacy assessment. CR rate was low (3%), and responses, though durable, were rarely complete. MZL subtype heterogeneity (nodal, extranodal, splenic) limited power for subtype-specific conclusions. Continuous indefinite dosing carries cumulative cardiovascular/bleeding toxicity risk.
Clinical Context
PCYC-1121 supported the FDA accelerated approval of ibrutinib for R/R MZL after ≥1 prior anti-CD20 therapy (2017), the first targeted agent approved in this setting. Newer covalent BTK inhibitors (zanubrutinib via MAGNOLIA) offer improved tolerability with less atrial fibrillation, and zanubrutinib has subsequently gained approval for R/R MZL.
