Background
International, open-label, non-randomised phase 2 (SPEARHEAD-1). N=52 in cohort 1 with HLA-A*02 and MAGE-A4-expressing advanced synovial sarcoma (n=44) or myxoid round cell liposarcoma (MRCL, n=8), age 16-75 yr, prior ≥1 anthracycline- or ifosfamide-containing regimen; 23 sites Canada/USA/Europe, Dec 2019-Jul 2021. Afamitresgene autoleucel (afami-cel) is an autologous TCR-engineered T-cell product targeting MAGE-A4 on HLA-A*02. Median 3 prior lines. NCT04044768.
Interventions and follow up
Treatment: Afamitresgene autoleucel (afami-cel; MAGE-A4-directed autologous T cells) — single IV infusion after lymphodepleting chemotherapy (fludarabine + cyclophosphamide)
Primary endpoint: Overall response rate by masked independent review committee per RECIST 1.1 in the modified ITT population
mFollow up: 32.6 mo (IQR 29.4-36.1)
Results
ORR (all): 37% (19/52; 95% CI 24-51%)
ORR (synovial sarcoma): 39% (17/44; 95% CI 24-55%)
ORR (MRCL): 25% (2/8; 95% CI 3-65%)
Cytokine release syndrome: 71% (37/52); all grade 1-2 except one grade 3 event
Adverse events
Grade ≥3 cytopenias (lymphodepletion-attributable): lymphopenia 96%, neutropenia 85%, leukopenia 81%; anaemia frequent
Neurotoxicity (ICANS): limited
Deaths: no treatment-related deaths
Management: prolonged cytopenias were the most common serious grade ≥3 events, requiring growth-factor support
Conclusions
Afami-cel achieved a 37% ORR in heavily pre-treated advanced synovial sarcoma and MRCL with durable responses at 32.6-mo follow-up and a manageable safety profile beyond lymphodepletion-related cytopenias — the first phase 2 trial to show meaningful TCR-T-cell efficacy in solid tumours.
Key Limitations
Single-arm, non-randomised phase 2 with surrogate ORR endpoint and small subgroups (MRCL n=8). Eligibility restricted to HLA-A*02 and MAGE-A4-expressing tumours, limiting applicability; lymphodepletion-related cytopenias and CRS require specialized cell-therapy infrastructure.
Clinical Context
Supported FDA approval (Aug 2024) of afami-cel for HLA-A*02-positive, MAGE-A4-expressing advanced synovial sarcoma after prior chemotherapy — the first engineered TCR-T-cell therapy approved for a solid tumour. Requires HLA/antigen screening and apheresis/manufacturing logistics, positioning it for later-line synovial sarcoma at specialized centres.
