Conclusions
Ivonescimab represents a novel second-line option addressing the limitations of single-agent checkpoint inhibitors in EGFR-mutated disease. Bispecific targeting of PD-1 and VEGF may overcome immunotherapy resistance mechanisms in post-TKI progression.
Key Limitations
Narrative review, not a systematic analysis — subject to selection and interpretation bias. HARMONi-A data are predominantly from a Chinese population, limiting generalizability. Ivonescimab not yet broadly FDA/EMA approved at time of review. No new primary data; sequencing recommendations are expert opinion pending randomized confirmation.
Clinical Context
First-line standards for EGFR+ NSCLC include osimertinib monotherapy, osimertinib + chemotherapy (FLAURA2), and amivantamab + lazertinib (MARIPOSA). Post-TKI options per ASCO and ESMO include platinum-doublet chemotherapy with or without amivantamab. Ivonescimab + chemotherapy is an emerging second-line approach; this review frames its potential role pending broader regulatory approval and Western data.
