Background
Phase III RCT (MPACT). 861 patients with previously untreated metastatic pancreatic adenocarcinoma. Randomized 1:1 to nab-paclitaxel + gemcitabine versus gemcitabine alone. Established the nab-paclitaxel + gemcitabine combination as a standard first-line regimen.
Interventions and follow up
Arm A: nab-Paclitaxel 125 mg/m² + gemcitabine 1,000 mg/m² days 1, 8, 15 every 4 weeks
Arm B: Gemcitabine 1,000 mg/m² weekly ×7 then weekly ×3 every 4 weeks
Primary endpoint: Overall survival
Median follow-up: 13.9 months
Results
mOS: 8.5 months (nab-P + gem) vs 6.7 months (gem), HR 0.72, 95% CI 0.62–0.83, P<.001
mPFS: 5.5 vs 3.7 months, HR 0.69, P<.001
ORR: 23% vs 7%, P<.001
1-yr OS: 35% vs 22%
2-yr OS: 9% vs 4%
Adverse events
Hematologic: Grade ≥3 neutropenia 38% vs 27%.
Non-hematologic: Grade ≥3 fatigue 17% vs 7%, peripheral neuropathy 17% vs 1%, nausea 6% vs 3%; no treatment-related deaths.
Conclusions
nab-Paclitaxel + gemcitabine significantly improved OS (8.5 vs 6.7 months, HR 0.72), PFS, and response rate versus gemcitabine alone in metastatic pancreatic cancer, establishing it as a standard first-line regimen, particularly for patients with PS 0-2 unable to tolerate FOLFIRINOX.
Key Limitations
Modest absolute OS benefit (~1.8 months). No head-to-head comparison with FOLFIRINOX. The proposed SPARC-mediated mechanism was not validated as a predictive biomarker. Cumulative peripheral neuropathy (17% grade ≥3) may limit long-term therapy. The benefit versus gemcitabine may be smaller when gemcitabine dose intensity is optimized.
Clinical Context
MPACT and ACCORD 11 (FOLFIRINOX) are both first-line standards for metastatic pancreatic cancer (per ESMO/ASCO). FOLFIRINOX is preferred for PS 0-1 patients without biliary stents; nab-paclitaxel + gemcitabine for PS 1-2 or patients with comorbidities. No prospective head-to-head comparison exists. Second-line options differ (oxaliplatin-based after FOLFIRINOX failure; FOLFIRINOX after nab-P + gem).
