Radiation OncologyHead and Neck CancerThyroid2020
Background
Retrospective single-institution cohort. Anaplastic thyroid carcinoma (ATC) — the most aggressive thyroid malignancy — treated with multimodality therapy (surgery, chemoradiotherapy, systemic therapy). Analyzed survival and failure patterns under curative-intent multimodality treatment.
Interventions and follow up
Treatment: Concurrent chemoradiation, with surgery and systemic therapy as part of multimodality care
Primary endpoint: Locoregional control, overall survival, toxicity
mFollow up: NR
Results
Median survival: Poor overall (typically 3-6 months in most series)
Resection effect: Complete resection + adjuvant CRT associated with longer survival than unresectable disease
Failure patterns: Both locoregional failure and distant metastases (lung predominant) are major
Adverse events
Mucosal/skin: Hyperfractionated accelerated RT + concurrent chemo causes significant mucositis, dysphagia, skin reactions
Airway: Tracheostomy often required
Overall: High toxicity burden from treatment intensification
Conclusions
Multimodality treatment (surgery, chemoradiotherapy, systemic therapy) can achieve disease control and prolonged survival in selected ATC patients, though prognosis remains poor. Complete resection followed by adjuvant chemoradiotherapy offers the best outcomes in resectable cases.
Key Limitations
Retrospective cohort with selection bias; small numbers inherent to rare ATC; no randomized comparison; survival figures cited as general series ranges rather than detailed cohort-specific numbers in source.
Clinical Context
Reinforces aggressive multimodality therapy for resectable ATC. BRAF V600E-mutant ATC is now treated with dabrafenib + trametinib (FDA-approved), often as neoadjuvant to enable resection; this shifts the modern paradigm beyond chemoradiation alone.
